Local Social Services
At Madison Health, our Social Services team helps you and your family handle any concerns that you may have during your inpatient stay. We understand the impact of illness and related challenges on patients’ and their loved ones’ lives. Our experienced and caring clinicians help people address challenges they may be experiencing.
Our Services
Our social workers can provide information about hospital and community resources, make specific arrangements and provide services such as:
Social Services
Assist patients and families with adjusting to illness and hospitalization
Assist patients with financial concerns
Mediate in crisis situations
Provide information and referrals on community resources, agencies and services
Help with grief, loss and bereavement issues
Assist in discharge planning
Assist with placement in a nursing home, rehabilitation facility or assisted living
Arrange for in-home services/referrals
Arrange for home medical equipment supplies
Assist with Advance Directives such as living wills, durable power of attorneys, power of attorneys
Assist in protection of persons of all ages
